Examples of images for an incident

There is no limit to the size or file-type of image that can be uploaded to the Hasmate program.  Suggested file types are .jpg, .jpeg, .gif, or .png.

A suggestion of what you might add to an incident are:

  • A photo of the equipment/machinery/vehicle/etc involved in the incident, and any damage.

Examples of attachments for an incident

There is no limit to the size or file-type of the attachments that can be uploaded to the Hasmate program.  Suggested file types are .docx, .doc, .pdf, .xls, or .xlsx.

Some suggestions of what you might add to an incident are:

  • A copy of any communications with WorkSafe, etc;
  • A copy of any ACC information;
  • A copy of doctors’ certificates;
  • A copy of any investigation (internal or external) communications;
  • A copy of any external expert reports and recommendations;
  • A cost schedule or report; or
  • Insurance reports.
